PARTICIPATION AGREEMENT: By signing below, the participant or legal guardian acknowledges and accepts the risks of physical injury associated with the activities connected to Parkview Christian Church (PCC). Except for the gross negligence of PCC, the participant or legal guardian promises to hold harmless PCC and its representatives for any accident or injury related to the activity. The participant or legal guardian also authorizes PCC to use any pictures that may be taken at this activity for the use of promoting future PCC activities.
